How do I report incidents, hazards and near misses?

To keep you safe at work you need to let us know about any hazards and near misses and for you to report all employee incidents.

 

Here’s how to report employee hazards and near misses:

A hazard is anything in the workplace that has the potential to cause harm and if left untreated may cause injury to employees and clients. Spotting and reporting hazards is part of everyone’s responsibility and hazards come in many forms for example:

  • Equipment that does not work properly
  • Alleged abuse occurring in a household
  • A dog that tries to bite
  • Clients who smoke in their homes when employees are on shift
  • Uneven steps
  • Slippery floors and loose mats
  • Loose handrails in a client’s bathroom
  • Clutter in a client home which impedes walkways
  • A near miss
  • Lack of equipment

To report an Employee Hazard or near miss:
  • Contact the Manager, Supervisor, Coordinator or Team Leader
  • Phone the Service Centre on 1800466300 or 1800243232 and ask to speak to Safety
  • Or record the Hazard or near miss using Confluence

 

Here’s how to Report Employee Incidents:

 An employee incident is an instance of something occurring in the workplace, an event or occurrence where an injury may have occurred for example:

  • A niggling pain which does not resolve
  • A motor vehicle accident when at work
  • Abuse at a client home
  • An injury resulting from complex manual handling
  • An injury as a result of a slip, trip or fall
  • A needle stick injury or contact with blood and or bodily fluids
  • Cuts and lacerations

 

To report an employee incident:
  •  Contact the Manager, Supervisor, Coordinator or Team Leader
  • Phone the Service Centre on 1800466300 or 1800243232 and ask to speak to Safety
  • Or record the Hazard or near miss using Confluence
