Tasmanian Home and Community Care (Tas HACC) Services

The Tasmanian Home and Community Care (Tas HACC) Program provides support for eligible people who need assistance with daily living activities. The program is funded by the Tasmanian Government and aims to help people maintain their independence and continue living safely at home.

Tas HACC supports eligible people under 65 who are living with a functional disability, chronic illness or mental illness and need ongoing assistance to maintain their independence at home. Other eligibility requirements also apply, and your individual circumstances will be considered when you are referred to the program.

Depending on your individual needs, Tas HACC support may include personal care, domestic assistance, shopping, meal preparation, social support, post-acute care following a hospital stay or period of ill health, and assistance with maintaining your home.

To access the Tas HACC Program, you will need a referral through the Tasmanian Community Care Referral Service. You, a family member or carer, a health professional or another service provider can make the referral.

You can request Anglicare Tasmania as your preferred Tas HACC service provider when going through the Tasmanian Community Care Referral Service. Referrals are issued to available service providers, and Anglicare will contact you once your referral has been received.
