Care Coordination Service

For people in alcohol and drug programs

Anglicare Tasmania’s state wide Care Coordination Service coordinates services for people who have significant alcohol and drug issues and one or more other challenges which can include:

  • an acquired brain injury
  • mental or physical health issues
  • intellectual or cognitive impairments
  • risk-taking behaviours
  • at risk of homelessness
  • factors in their environment that pose a risk (such as family violence).

Together with you, your care is coordinated to ensure the most appropriate services are provided. You are supported to play an active role in the decision-making.

How to access

To access the Care Coordination service you must be referred through the Alcohol and Drug Services (ADS) by you, your GP or by another service or health professional.

ADS is a free alcohol and drug treatment service funded by the Tasmanian Government Department of Health.  They have clinics in Hobart, Launceston and Ulverstone.

Care Coordination is supported by the Crown through the Department of Health (Tasmania).

Contact ADS

If you would like to be referred, or refer someone to the Care Coordination Service, you need to contact Alcohol and Drug Services.

They require an Intake and Referral Form to be completed. You can find the form here.
